Forms required for prospective students:
*Please note: We cannot accept tax forms and W-2 forms that have been emailed. Tax forms and W-2 forms that have been emailed must be deleted for privacy and security purposes.
- UChicago Financial Aid Worksheet (can be completed at no cost via the applicant’s UChicago Account)—or optional CSS Profile. Please note that you cannot upload financial aid documentation through the CSS Profile IDOC system for UChicago Financial Aid decisions.
- Free Application for Federal Student Aid (FAFSA) (School code 001774)
- Parent 2021 Federal Tax Return (including all schedules)
- Parent(s) W-2 forms. If your parent(s) did not receive a W-2 form please reach out to our office.
- Parent 2021 Federal Business tax returns (if applicable) including all schedules. These are required regardless of percentage ownership that parents hold in the business.
Prospective students are encouraged to upload documents through their UChicago Account. This is the fastest and most efficient way to upload a variety of financial aid documents. While the majority of requested documents can be submitted using this upload feature, please note that there are some documents that cannot be uploaded. Please review your UChicago account for a list of documents that can be uploaded.
Students can now begin submitting documents for 2023-24 financial aid renewal. Please reach out to our office if:
- You would like to apply for financial aid but do not see any financial aid requirements in your My.UChicago Portal
- You are only interested in receiving your merit financial aid and not applying for need-based financial aid.

Returning Student Priority Dates

Forms needed for rising 2nd, 3rd & 4th year students: Applications and materials submitted after the priority will be reviewed and all eligible aid will be awarded, however it may be after the first bill has been released.

- Free Application for Federal Student Aid (FAFSA) (School code 001774)
- Parent 2021 Federal Tax Return: This may be mailed or faxed to the Office of College Aid. Taxes must be signed and include the student ID number. Include all schedules.
- Parent(s) W-2 forms. If your parent(s) did not receive a W-2 form please reach out to our office.
- Parent 2021 Federal Business tax returns (if applicable) including all schedules. These are required regardless of percentage ownership that parents hold in the business.
Federal Financial Aid
If you want to renew federal funds only, like Stafford Loans or the Pell Grant programs, submit the documents below. You should also follow these directions if your family needs to borrow a Federal Direct Loan Parent PLUS Loan.

Merit-Based Aid
If you have received merit-based aid during the admissions process, it will automatically be added to your package. You do not need to submit any documentation. If your merit scholarship is recurring, it will be part of your package each year.
International Financial Aid
Financial aid eligibility for international students is determined prior to their first year of enrollment and their packages will not change in subsequent years. Nothing is required to renew International student financial aid.
Federal Direct PLUS Loan Only
You did not apply for financial aid but your family wants to borrow using the Federal Direct PLUS Loan.
Forms Needed:
- Free Application for Federal Student Aid (FAFSA) (School code 001774)
- Complete the PLUS Application and obtain a PLUS credit approval
- Complete Master Promissory Note
