Forms required for prospective students: 

  • UChicago Financial Aid Worksheet (or optional CSS Profile – please note that you cannot upload financial aid documentation through the CSS Profile IDOC system for UChicago Financial Aid decisions)
  • Free Application for Federal Student Aid (FAFSA) (School code 001774)
  • Parent 2016 Federal Tax Return (including all schedules) and W-2 forms
  • Parent 2016 Federal Business tax returns (if applicable) including all schedules. These are required regardless of percentage ownership that parents hold in the business.

Prospective students are encouraged to upload documents through their UChicago Account. This is the fastest and most efficient way to upload a variety of financial aid documents.  While the majority of requested documents can be submitted using this upload feature, please note that there are some documents that cannot be uploaded. Please review your UChicago account for a list of documents that can be uploaded.

You may fax documents to 773-702-0744 or mail to: Office of Financial Aid, 1115 E. 58th Street, Chicago, IL 60637. When faxing documents to our office, please use the Financial Aid Cover Sheet.

Returning Students+

Returning Student Priority Dates 

  Summer Federal Work-Study To have aid decision prior to first bill
Priority deadline for document submission March 31 May 31

Forms needed for rising 2nd, 3rd & 4th year students:Applications and materials submitted after the priority will be reviewed and all eligible aid will be awarded, however it may be after the first bill has been released.  

  • UChicago Financial Aid Worksheet
  • Free Application for Federal Student Aid (FAFSA) (School code 001774)
  • Parent 2016 Federal Tax Return: This may be mailed or faxed to the Office of College Aid. Taxes must be signed and include the student ID number. Include all schedules and W-2 forms
  • Parent 2016 Federal Business tax returns (if applicable) including all schedules. These are required regardless of percentage ownership that parents hold in the business.

*If you were accepted to the University of Chicago prior to the 2015-16 academic year you must complete the CSS Profile. In addition, if your parents are divorced, separated, or were never married, your non-custodial parent must complete the Non-Custodial Parent (NCP) Profile on the College Board website. If your family submitted an appeal to have this requirement waived and your appeal was approved by the Office of College Aid, your family is not required to complete the NCP Profile.

The preferred method of sending documents to the Office of Financial Aid for returning students is via fax to 773-834-4300. However, you may also mail documents to the Office of Financial Aid at 1115 E. 58th Street, Chicago, IL 60637. Please use the document cover sheet to submit information via fax or mail. When faxing documents to our office, please use the Financial Aid Cover Sheet.

Special Circumstance Appeals

At UChicago, we realize that your financial aid application may not always reflect the realities of your family’s financial situation. We use your family’s prior-prior year income to determine your financial aid eligibility, and situations can change in the interim. Because of this, we offer students and families an opportunity to appeal their financial aid decision if one of the following special circumstances exists:

  • Loss or reduction of employment, significant reduction of income (job loss with 12 weeks of continuous unemployment, death of parent, etc.)
  • One-time, nonrecurring income (pension payout, inheritance, employer reimbursements) that artificially inflated the adjusted gross income for the year reviewed
  • Out-of-Pocket medical or dental expenses in excess of 11% of your parent(s) Adjusted Gross Income

If you believe that your family’s financial situation has been impacted by one of these circumstances (or some variation thereof) please complete and submit to our office the Special Circumstances Request Form and provide the required documentation. While submission of a special circumstance request does not guarantee an increase in your financial aid, we will thoroughly review your situation to determine if any additional aid could be made available.  

Federal Financial Aid

If you want to renew federal funds only, like Stafford Loans or the Pell Grant programs, submit the documents below. You should also follow these directions if your family needs to borrow a Federal Direct Loan Parent PLUS Loan.

Merit-Based Aid

If you have been awarded merit-based aid during the admissions process, it will automatically be added to your award. You do not need to submit any documentation. If your merit award is recurring, it will be awarded each year.  

Questbridge

To renew your Questbridge scholarship, please follow the directions below.

Forms Needed:

International Financial Aid

Your international financial aid award will be automatically renewed.

Federal Direct PLUS Loan Only

You did not apply for financial aid but your family wants to borrow using the Federal Direct PLUS Loan.
