2017-2018 Budget Appeal Form

   

Financial Aid determines financial assistance based on the standard cost of attendance for a single student in a degree program. Under certain circumstances, you may be able to borrow additional funds if needed.

2017-2018 Satisfactory Academic Progress Appeal

   

This appeal must be completed by the deadline date listed below in order to receive Title IV Federal student aid for the quarter in which aid was suspended.

General Purpose

Alternative Loan Request Form

   

The Alternative Loan is a non-Federal loan. The lenders provide specialized student loans with reasonable terms for International students (who have a credit-worthy, U.S. citizen cosigner), as well as U.S. citizens, nationals, and permanent resident who decided not to borrow from the Federal loan program.

Consortium Agreement

   

Degree-seeking students who will attend another (domestic) academic institution and whose credits will transfer towards their degree at the University of Chicago must have a financial aid officer at the other institution complete the consortium agreement and return it to our office. Your Dean of Students must approve this arrangement and can answer your questions.

Limited Power of Attorney Form

   

You may authorize another person to pick up a student loan check on your behalf if you are unable to come to campus. This form must be notarized.

Loan and Federal Work-Study Change Form

   

To request additional funding or cancel existing loans and/or Federal Work-Study funds, complete the Loan and/or Federal Work-Study Change Form and submit the form to Financial Aid

Pre-Med Preparatory Coursework Loan Request

   

In addition to completing the Free Application for Federal Student Aid (FAFSA), pre-med students must complete the Pre-Med Preparatory Coursework Loan Request form. Pre-med students must list the courses in which they will enroll and have their academic adviser or an authorized individual from their department confirm the necessary preparatory courses and sign the certification.

Student Release of Information

   

Use this optional form if you want to authorize Graduate Financial Aid to discuss your financial aid information with a third party, such as your spouse, parent, landlord, etc. We cannot release information specific to your financial assistance without your written permission, which will be valid from the date you submit your release form through the last day of spring quarter, when the school year ends.
